Weasel Hag

Covetous and bedecked in stolen and gifted teeth, weasel hags are Fey creatures that sow nightmares and temptation to wickedness. They often surround themselves with weasels and boogleswarms, using them as spies, thieves to steal trinkets (especially lost teeth), and to spread their malign influence.
The hag has Advantage on Reaction Rolls against spells and other magical effects.
If a creature willingly offers one of its teeth to the hag, the hag must accept it and has the Vulnerable (Charmed) condition for 24 hours, or until the creature or its allies deal damage to the hag or force it to make a Reaction Roll.
The Weasel Hag can be spotlighted up to 2 times per GM turn. Spend Fear as usual.
A beam of enervating energy shoots from you toward a creature within Close Range. The target must make a DC13 Strength Reaction Roll. On a successful roll, the target has Disadvantage on the next attack roll it makes until the start of the GM's next turn.
On a failed save, the target has Disadvantage on Strength-based rolls for the duration. During that time, it also subtracts 1d8 from all its damage rolls. The target repeats the roll at the end of each of its turns, ending the spell on a success.
A target that the Hag can see within Close Range must make a DC13 Knowledge Reaction Roll. If the Hag has one of the target's teeth, that target has Disadvantage on the roll. On a failure, the target takes 3d6 physical damage, and now has the Frightened condition.
While Frightened in this way, the target perceives its allies as enemies born from its nightmares, but it isn’t compelled to act against or harm them. At the end of each of its spotlight, the target repeats the roll, ending the effect on itself on a success. After 2 GM turns, it succeeds automatically. Success: Half damage only.
The hag shape-shifts to resemble a tiny weasel or a Small or Medium Humanoid, or back into her true form. Her game statistics, other than her size, are the same in each form. Any equipment she is wearing or carrying isn’t transformed.
